Religious exemption forms do not expire. When forms are updated, parents do not have to re-submit an updated religious exemption form. Older forms are grandfathered in.

Forms accepted:

  • Prior to August of 2015 - any form on file is accepted.
  • August 2015 through February 2018 - form 2208, no DOB, dated May 2015
  • March 2018 through May 2019 - form 2208, with DOB, dated March 2018.
  • June 2019 forward - form 2208, with DOB and dated June 2019

Updated forms are often times sent out prior to the date on the updated form and would be accepted within the older date range. For example, you have a form signed in May 2019, but it is on the newest (June 2019) form. This is acceptable.

Remember, religious exemptions must be notarized, have a visible stamp, and have no scratch outs/mark throughs.

All prescription medication must be in its original container. Any student caught distributing, selling, exchanging, or attempting to ingest prescription medication will be brought before a disciplinary tribunal.

Students will be allowed to carry their own non-prescription medication. Non-prescription medication must be in its original container. Students are prohibited from giving non-prescription medication to other students. Any student caught distributing, selling, or exchanging non-prescription medication to another student may be brought before a disciplinary tribunal.
